Attention Parents,
This form is only required for parents who are requesting for their children to attend a school they are not zoned to attend.  All other parents may disregard.  Out of zone and out of district parents are required to submit this form no later than May 1, 2026. Be sure to log into your ParentSquare account so that student data will auto-populate on the form. All decisions will be communicated by June 1, 2026. A printable copy of the document is attached as well and is available at all district schools from April 1-May 1, 2026. 

NEW applicants will not be considered without a copy of student records (including grades, discipline, attendance, and IEP/504 data) with their completed application. Records may be emailed to mandie.fowler@lcsb.org or dropped off at any district school. New applicants MUST use the attached Word document & will not be able to use the ParentSquare form. New applicants will be required to complete additional paperwork and/or follow-up pending acceptance.
